Community Health Action is committed to
transforming lives through prevention and care. We reach thousands
of people a year through our helpline, targeted outreach, social and
educational programs, and high school trainings, which dispense
important prevention and medical information about HIV/AIDS and
other STDs and blood-borne diseases.
The Health Education and Prevention department at
Community Health Action provides education to the community by
facilitating workshops, trainings, and educational presentations.
Staff members, trained peer staff and volunteers offer information
about HIV/AIDS prevention, LGBT issues, and resources available to
Staten Island through community outreach, small group interventions,
and counseling.
The agency has 10 case
management teams for HIV+ clients, with four specialized teams to
serve seriously mentally ill persons with HIV, HIV+ undocumented
immigrants, HIV+ men in NYSDOCS Arthur Kill Correctional Facility,
and women with HIV+ or sero-reverted children. We provide
intensive support to HIV+ NYC-based releases from Northeastern NY
State correctional facilities by meeting them immediately upon their
release and escorting them to services. Other Client Services
include support groups, individual and family counseling, housing,
regular recreational and cultural events, substance abuse treatment,
escort and translation services, job counseling and an emergency
food pantry.
