Health Home
Community Health Action of Staten Island’s Health Home program provides comprehensive care management services for people with multiple chronic health conditions or mental health needs. If you need help coordinating your care for diabetes. asthma, hypertension, or mental health conditions like schizophrenia, anxiety or major depression give us a call.
How We Can Help
If you have one or more illnesses with ongoing medical needs, our Health Home program can help you find the right care, link you to services and benefits for which you may be eligible, and help you find the best up-to-date educational materials about your condition and ways to feel better.
Though we don’t provide housing, if you are a health home member in an unstable situation or in danger of losing your housing, your care manager can help you find city relief programs and options for stability.
What to Expect (How Health Home works)
You meet with our intake specialist who will ask questions about your needs and your history (in person or by phone/ video)
The worker will work with Medicaid to find out if you qualify and to make sure you have no gaps in your care while we enroll you. This may take a few days.
A care manager is assigned to you, and will call you to schedule a first appointment will get to know your history, your challenges, and your ongoing health needs
With your consent, your health records can be shared among your health care providers so that services are coordinated to help you. We take a team-based approach with your network of care providers.
Your care manager will help everyone on your team stay connected about how you’re doing, and they will make sure you feel supported and that you’re all working toward the same goals for your health.
We want you to stay engaged with the care you need most. We will help coordinate appointments and address anything that is getting in the way of your continued improvement and independence.
You can also expect your care manager to
- Provide you with information, education, and skills to manage your health and meet your health goals
- Help you get doctor’s appointments and medical care like dentists and counselors if you need it
- Help you understand your medications and the treatments that your doctors prescribe
- Assist you with accessing benefits (SNAP, Disability, HRA, and more)
- Help you solve problems and remove barriers that may prevent you from getting the care you need like transportation to appointments
