CHASI’s Core Connections program (CoCo) connects Staten Islanders with Medicaid to a variety of services that support healthy living. Some participants may be eligible for brand new services like housing support, home delivery of medically-tailored meals, transportation support, and more.

New York State’s Social Care Needs (SCN) Network

New York state uses Medicaid funding to support a wide range of supportive services that address barriers to health care access. CHASI is part of a social care network managed by the Staten Island Performing Provider System (SI PPS). This network is made up of nonprofit organizations, health care providers, and other community-based organizations who all provide free services for Staten Islanders. We work with you to identify your priorities and then work together to coordinate your care. Staff in this program are equipped with information about local resources and how to access them, and they can help you navigate the applications and inquiries so you don’t have to do it alone. We work collaboratively to get you what you need and we report back to the PPS and Medicaid about which services are still needed. You can always count on CHASI to connect you with reliable health information, and now we can also make sure you get direct local access to people who are available to help with benefits access, care for chronic diseases, supports for adults and children with dietary needs, education and job training, and more.
