CHASI can help you figure out where to begin if you have just found out you are HIV positive or how to pick up if you are re-entering treatment for HIV.
Community Health Action has been at the forefront of HIV and AIDS care services in Staten Island for over 30 years. Our clients benefit from our network of doctors and providers, and from our outstanding patient navigators who can help you coordinate multiple services and benefits. You don’t have to face finding critical services on your own. We offer group workshops and connections to specialized providers like dentists and other providers who understand how HIV can impact your health (and how it doesn’t impact your health!). We will also advocate for you to find or remain in housing that helps you stay healthy. You can meet with your care worker on a weekly or daily basis, depending on the level of care you need. Our goal is to help you gradually progress toward independence with managing your own health care and treatment.

CHASI’s HIV Care Coordination team collaborates with anyone who’s helping you. We can help you if you have experienced trauma or violence, food insecurity, difficulty disclosing your status to a partner, care for STIs or other medical needs, and more.
