Your Support Impacts Lives

Community Health Action of Staten Island serves approximately 19,000 people through programs for people with HIV and other chronic illnesses, STI prevention and testing, domestic violence services, alternatives to incarceration for people involved with the criminal legal system, food & nutrition programs, and community-based health education services. CHASI sites are in St. George and Port Richmond, offices in the SI Family Justice Center, in NYCHA residences, a satellite office in Queens, and we visit neighborhoods across Staten Island with our mobile food pantry and mobile health unit.

Domestic Violence & Trauma-Based Services

Your support ensures every person seeking help after violence is connected with a crisis counselor. CHASI partners with the Staten Island Family Justice Center to provide crisis counseling and case management support to victims of domestic violence. Our counselors use a trauma-informed approach to domestic violence, helping individuals and families connect with services before, during, and after criminal justice intervention. Individual counseling and group therapy services are provided in English and Spanish.

Community Access

Your support puts food on the table for thousands of Staten Islanders each year and ensures no one goes hungry. CHASI’s Community Access department connects Staten Islanders in need with free nutritious food, connections to SNAP and health insurance benefits, and a variety of resources for medical and mental health. The department includes a storefront pantry in Port Richmond, a mobile food pantry that visits neighborhoods across the island, and a linkage and navigation program to facilitate connecting people in need with a variety of CHASI Services.

PEP, PrEP, Prevention,& Testing

Your support ensures that Staten Islanders of every generation have safer sex and prevention education and access to medications and tools to End the HIV epidemic in NY. HIV Prevention has changed significantly since we began this work over thirty years ago. Community Health Action remains committed to our roots in HIV prevention, testing, and education. In addition to condoms and safer sex practices, medication that prevents HIV can be prescribed to people who want to lower their risk.

Behavioral Health

Your support keeps our doors open 24 hours a day, 365 days a year for people seeking to make meaningful change in their relationship to drugs and alcohol. Community Health Action provides a continuum of care for people harmed by substance use. We have "no wrong door" approach -- we work with people who use drugs, those seeking options to change or stop their use, and those in recovery. Clients have access to counseling and support from clinicians, recovery coaches, and peers.

Care Management

Your support removes barriers between our neighbors and the care they deserve. More than 40% of New Yorkers live with a chronic health condition. Community Health Action helps people with complex medical needs take control of their health, access high quality care, and become less reliant on emergency services. CHASI care managers help clients navigate the health care system – finding doctors, keeping appointments, adhering to medication regimens, and centralizing care to focus on getting well.
