CHASI’s Health Equity team is dedicated to reducing the impact of social factors like racism and inequality on health outcomes. Across Staten Island, there are thousands of people living with undiagnosed and untreated hypertension and diabetes. We offer free health screenings to anyone who wants to take control of their health. Our outreach is focused in places where we gather with neighbors to talk about the importance of routine screenings, like barbershops, community centers, and housing developments. Many screenings are offered on the spot, and others are accompanied with health coaching and follow ups to keep you healthy.

Black men in the U.S. have higher rates of uncontrolled high blood pressure and other health conditions in part because of social and environmental factors like racism, stress, and barriers to accessing health care. Health Equity is when everyone has the opportunity to achieve their full health potential, no matter their background and circumstances.
Located in the Playsure Health Hub at 26 Bay Street, CHASI’s Health Equity program can help you manage your hypertension through early intervention, lifestyle coaching, and regular check ins to help you stay on track with healthier eating and activity.
How We Can Help
We offer workshops, education, information, and screening about hypertension (high blood pressure), one of the most prevalent conditions in New Yorkers. Health educators offer screening services if you don’t know if your risk factors. If you are at risk or are identified with diabetes or high blood pressure, we can help you get access to quality medical care, information about your condition and ongoing support. Our staff will also help you plan and make lifestyle changes to adapt to your condition, and help you find ways to keep it under control.
Health Equity program members receive 1:1 health & wellness coaching and support like:
- Blood pressure readings
- Coaching on how to talk to your doctor to get optimal care
- Help getting to your appointments
- Access to healthy eating and nutrition workshops
